REQUEST RESIGNATION OF OFFICER
RESOLVED, that [Officer's Name] is hereby requested to resign as [Job Title] of the Corporation, effective immediately, and [Secretary's Name] the Secretary of the Corporation is hereby directed to send a copy of this resolution to the said [Job Title].
The undersigned hereby certifies that he/she is the duly elected and qualified Secretary and the custodian of the books and records and seal of the corporation above-named. The undersigned also certifies that this corporation has been duly formed in accordance with the laws of [Jurisdiction].
